Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, and most major credit cards. Payment is expected at the time services are performed. If your treatment plan requires more comprehensive dental work, third party financing is available.
Dental Insurance
We welcome most major dental insurances at this office!

of limitations imposed by your employer on your insurance plan. We will
always provide you with our best service.
We will always strive to ensure you get the maximum benefit you are entitled under your dental plan.
Your dental plan's coverage is based on an agreement between your
employer and an insurance company. It may or may not cover the full fee
for treatment. As a courtesy we will process your claim. We will provide
any necessary documentation to aid in the processing of your claim.
